The Office of Students in Transition identifies children and youth in vulnerable situations and removes any barriers that would prevent them from attending school.
The term “homeless children and youth” means individuals who lack a fixed, regular and adequate nighttime residence.
If you are in transition, please complete the two forms below and return to one of the email addresses listed below.

Complete the McKinney-Vento Referral Form and Louisiana Student Residency Questionnaire Form for each student if qualified for assistance.
Fax the forms to 225-456-2858 or email the forms to one of the following:
